Biohacking Energy: What Is NAD+ and Why Wellness Fans Love It
Spend enough time in wellness circles and you will hear NAD+ described as an energy switch, an anti-aging molecule, or the next step in biohacking. It appears in IV lounges, supplements, podcasts, and morning-routine videos. Biology is real. The promises attached to it often run ahead of the evidence.
NAD+ stands for nicotinamide adenine dinucleotide. It is a coenzyme found in every cell and is essential for energy metabolism, redox reactions, DNA repair, and other forms of cellular maintenance. One point is easy to miss: NAD+ is not a peptide. It is a different type of molecule, although wellness businesses often discuss it alongside peptides.
NAD+ metabolism changes with age and disease, which has led researchers to ask whether restoring NAD+ availability could improve health. That is a reasonable scientific question. It is not the same as proving that an infusion or supplement slows human aging.
What human studies tell us
Most clinical studies have tested precursors that the body can use to make NAD+, particularly nicotinamide riboside, or NR, and nicotinamide mononucleotide, or NMN. A 2018 randomized trial found that NR was well-tolerated and increased NAD+ metabolites in healthy middle-aged and older adults. That confirms a biochemical effect.
The larger question is whether raising NAD+ produces meaningful improvements in energy, physical function, metabolic health, cognition, or lifespan. A 2023 Science Advances review examined the human NR literature and found that NAD+ availability can be increased, while consistent clinical benefits remained difficult to demonstrate. Trials were generally small, short, and conducted in different populations.
The evidence has continued to develop, but the distinction remains important. A biomarker can move without producing a benefit that a person can feel or that changes long-term health. Anti-aging claims should therefore be judged by clinical outcomes, not by NAD+ concentrations alone.
Oral precursors and injectable NAD+ also should not be treated as interchangeable. Oral NR and NMN have more published human trial data. Injectable and intravenous NAD+ products are not FDA-approved for energy enhancement or anti-aging. Compounded products are not reviewed by FDA for safety, effectiveness, or quality before marketing.
There is a specific quality concern with sterile injections. FDA has warned that ingredients labeled for food or supplement use are not automatically suitable for sterile compounding. The agency has received reports of severe chills, shaking, vomiting, and fatigue after injectable NAD+ products, with reactions consistent with excessive endotoxin exposure. That is a manufacturing and sterility issue, not a reason to dismiss NAD biology itself.
For the wellness reader, the practical takeaway is less exciting than the marketing. Short-term studies suggest that common oral precursors are generally well-tolerated, but long-term safety and clear benefits for healthy people are not settled. An IV is more intensive and expensive, but that does not make it more effective.
Yoga, regular movement, adequate sleep, and a varied diet do not sound futuristic, yet they have much stronger support for health than most biohacking packages. They also influence energy metabolism through many pathways rather than one fashionable molecule.
Anyone considering a supplement should review medications and health conditions with a qualified professional. Anyone considering an infusion should ask who prescribed it, who prepared it, whether the facility is appropriately licensed, what quality tests were performed, and what medical support is available if a reaction occurs.
NAD+ research is worth following because the molecule is central to cellular biology. It should be followed with the same discipline used in good yoga practice: attention, patience, and less attachment to dramatic promises.
